Abstract
Introducción: La migración ha aumentado en Chile y constituye un determinante social que impacta en el acceso a la salud. Dado que gran parte de la población migrante se encuentra en edad reproductiva, esta situación se refleja en los servicios de salud sexual y reproductiva, por lo que resulta relevante conocer las determinantes asociadas al acceso en su atención en el sistema sanitario. Objetivo: Comprender los determinantes de la búsqueda de atención en salud sexual y reproductiva de población migrante en la provincia de Iquique. Material y método: Estudio cualitativo exploratorio-descriptivo con perspectiva fenomenológica a través de los datos emanados del proyecto FONDECYT Regular 1220371. Con un muestreo teórico intencionado en bola de nieve se realizaron 23 entrevistas semiestructuradas a personas migrantes. Se utilizó la técnica de análisis de contenido a través de ATLAS Ti. Resultados: Las experiencias recogidas evidencian que la búsqueda o postergación de la atención en salud sexual y reproductiva responde a una interacción dinámica de diversos factores interrelacionados que según su naturaleza pueden facilitar u obstaculizar tanto el acceso como la permanencia en los distintos niveles de atención. Conclusión: Se evidencian características facilitadoras y obstaculizadoras para acceder a la atención en salud sexual y reproductiva, que dan cuenta de las complejidades del acceso, la calidad y la continuidad de la atención a la población migrante. Se propone un nuevo modelo de cinco niveles para comprender los factores que influyen en la búsqueda de atención en salud sexual y reproductiva y su permanencia en el sistema.
Introduction: Migration has increased in Chile and is a social determinant that impacts access to healthcare. Since a large portion of the migrant population is of reproductive age, this situation is reflected in sexual and reproductive health services. Therefore, it is important to understand the determinants associated with access to care in the healthcare system. Objective: To understand the determinants influencing the search for sexual and reproductive health care among the migrant population in the province of Iquique. Material and Methods: An exploratory-descriptive qualitative study with a phenomenological perspective based on data from the FONDECYT Regular Project 1220371. Using snowball sampling, 23 semi-structured interviews were conducted with migrants. Content analysis was performed through ATLAS Ti software. Results: The experiences gathered reveal that the search for or postponement of sexual and reproductive health care is influenced by a dynamic interaction of various interrelated factors, which, depending on their nature, can either facilitate or hinder both access and continuity of care at different levels. Conclusion: Facilitating and hindering factors for accessing sexual and reproductive health care are evident, shedding light on the complexities of access, quality, and continuity of care for the migrant population. A new five-level model is proposed to understand the factors influencing the search for sexual and reproductive health care and its continuity within the system.
